1. Who we are and what you can do here
Milieu License is operated by Milieunomics Institute. These terms cover our independent application preparation, record, optional download, payment and delivery services. We are not a municipality, county, government agency, or government-authorized filing agent. Official forms and government links identify their sources; they do not imply endorsement.
You can obtain official forms and apply directly with the responsible department without buying our services. Our guidance is general information, not legal, tax, engineering or other professional advice. Using the portal does not create an attorney-client relationship.
Use an account you control and sign only for yourself or with lawful authority. If you act for an organization or another person, you must have permission to provide their information and bind them where required. This service is intended for adults or users acting through an authorized adult.
2. Local and agency requirements
Applicable federal and Indiana law, the selected municipality’s or county’s code and adopted ordinances, and the responsible agency’s current requirements govern your application. Our summaries do not replace them. Code publications and forms can lag new ordinances; verify the current fee, form, deadline and submission method with the department.
Review every answer, required attachment and page of the completed official form. An on-screen check cannot establish eligibility or completeness. You may still need plans, inspections, professional licenses, original bonds, certifications, hearings, tests, witnesses or notarization. Do not begin regulated work until the necessary authorization is issued.
Each listed form identifies its municipality, county or shared department and links to the official source. Selecting a municipality can also show its county’s forms. A catalog listing does not mean every form is editable or eligible for automated delivery on Milieu License. Official online services, original documents and form directories are labeled separately. Electronic or postal delivery is application-specific. A listed email address alone does not establish acceptance of every form or signature method.
3. Electronic records, signatures and paper options
Signing is a deliberate, separate action. Review the document, enter your name or drawing, and select the signing consent before signing the identified version. The record includes your account, consent text and version, time and document fingerprint. Editing answers or attachments requires a new signature.
Electronic-record consent covers the application and its related signing records and service communications. You need internet access, a current browser capable of displaying these pages, an email account for communications, and a PDF reader plus storage or a printer to retain PDF copies. Review the preview and linked original form before signing. Contact us if you cannot access a record.
You may leave consent unchecked and choose paper signing or apply directly with the agency. To withdraw consent for future electronic records, update contact information, or request a paper or electronic copy, email info@milieupower.com. Withdrawal does not undo earlier valid signatures or recall deliveries; a service requiring electronic records may need a paper alternative. We do not charge for legally required record access. Optional convenience downloads have the separately disclosed price below; any optional printing or postage would be quoted before you agree.
Indiana’s Uniform Electronic Transactions Act (IC 26-2-8) and the federal E-SIGN Act (15 U.S.C. § 7001 et seq.) recognize electronic transactions subject to their conditions and exceptions. Agencies choose the electronic formats and signatures they accept. A typed name, drawing or document fingerprint does not verify identity or perform notarization. Follow the agency’s original-signature and notary instructions.
4. Language assistance and English paperwork
Choose a language for website text and help within the fillable form window. Translation runs on your device after the required language files download. Available languages and performance depend on the translation models, browser and device. You can return to English at any time.
When you use the form translator, narrative answers are translated into English for your review before saving. Names, addresses, identifiers and other exact entries are preserved. The reviewed English answers are used in generated downloadable, printed and mailed paperwork. Correct any translation that changes your meaning before signing or authorizing delivery.
The application records the languages and translation method used. Generated paperwork includes an English translation-assistance notice, including when an uploaded PDF is the selected primary document. Switching back to English does not remove the record of earlier translator use. The translation record is preserved with the signed application version.
Uploaded documents are kept as supplied and are not automatically translated. For delivery of a translated application, you confirm that the selected uploaded files are in English. Supply appropriate English documents or translations before including them. The on-screen translator is an assistance tool, not a certified translation or a human interpreter. It does not change official requirements, the exact payment you authorize, or your rights if our service is defective. Contact us for help correcting a translation or obtaining an accessible alternative.
5. Employment, grants and notarized forms
For the Mishawaka police application, sign the records-release page during the required notary process. Upload the completed notarized PDF when requested. The voluntary demographic survey is separate from the employment application and should be sent separately under the city’s instructions. Do not include it as a routine supporting attachment.
For grant applications, including CDBG, official eligibility, certifications, supporting records, original-signature requirements and deadlines still apply. Preparing or sending an application does not secure funding. A notary or other outside professional is a separate service; confirm qualifications, cost and agency acceptance before using it.
6. Clear prices and separate government fees

Automatic postal quotes show Milieu License’s retail price for the selected documents and delivery, not a quote of the provider’s costs. Printed page counts include any translation-assistance notice in the packet. Quotes are valid for 24 hours for the quoted selection. Changing the documents, destination or method requires a new quote. Calculating a quote does not charge you, print a check or send documents.
Checkout shows printing, mailing, preparation, any credit, the city check amount and the full total before you authorize it. Review and approve the exact payee, amount, mailing destination and documents. These are one-time services, not a recurring subscription. Provider readiness and printable-document limits still apply. 7. Cancellations, failed services and refunds
Processing or dispatch may begin as soon as payment is confirmed or a no-charge delivery is authorized. Check the packet and recipient before proceeding. Contact us promptly to request cancellation; a completed download or dispatched item cannot simply be recalled.
Request a full refund of our service charge before the paid work begins, for a duplicate charge, or if we cannot supply the purchased service. If a paid PDF is unavailable or materially defective, contact us for correction or a refund if we cannot provide it. Completed preparation is not refundable solely because the agency denies an application.
For incurred third-party delivery costs, any refund depends on recovery from the provider and applicable law. Government-fee refunds follow the agency’s rules. These terms do not limit remedies for an unauthorized charge, our failure to deliver a promised service, or other nonwaivable consumer rights. Refunds require support review; they are not automatic when you delete an application.
Request a refund or billing review with your application reference and receipt date. Do not email card numbers, bank credentials or sensitive attachments.
8. What delivery does and does not confirm
We may send only the packet, attachments, recipient, method and charges you approve in an enabled delivery flow. Review the exact selection, especially identification and financial documents. A checkout authorization is not permission for unrelated disclosure or another payment.
Standalone postal service remains available when city review or other application steps are unfinished. Choose the prepared document or uploaded PDF and attachments you want sent. Use Review required documents to check the application’s remaining steps.
A saved draft, electronic signature, payment receipt, email-delivery event or postal tracking update does not establish government acceptance, timely filing, fee credit, check clearance or approval. Confirm receipt and any remaining steps with the department. Only the agency can issue a permit, hire an applicant or award funding.
City review alone does not prevent an eligible check mailing. Your authorization and confirmed checkout payment cover the specified check and delivery; provider and bank readiness are checked before dispatch. When city instructions require payment after acceptance, mail the documents now and pay when invoiced. Existing orders funded separately retain their recorded funding terms. Production, mailing and delivery are separate events, and provider funding checks or other issues may delay dispatch.
Plan for processing, mailing and agency review time. Use the official route when a deadline is close or a feature is unavailable. This site does not replace emergency or immediate reporting channels.
Milieu Miracle facility orders
Milieu License also fulfills optional printing, mailing and check orders started on MilieuMiracle.org. Review and authorize the itemized facility quote there, including the exact signed packet, selected City or League payee, postal address and any check principal. Stripe processes checkout and Lob provides printing, mailing and check fulfillment. Milieu License preserves the selected packet version for that order; it does not change the signed facility forms.
Follow the order through Milieu Miracle using your private request code. Purchase and mailing may proceed while a facility request is under review, but payment, postal delivery, check acceptance and facility approval remain separate. Withdrawing the scheduling request does not automatically cancel a paid order or an issued check. 9. Your materials, privacy and responsible use
You retain your rights in the information and files you provide. You give us permission to handle them as needed for the services you request and as explained in the Privacy notice. Keep copies and submit only information you have authority to provide.
Do not impersonate someone, forge a signature, submit false information, access another person’s records or upload malicious files. We may restrict access where reasonably necessary to address misuse, protect records or comply with law. Contact support if you believe a restriction is mistaken or need access to your own records.
Our original design and content belong to Milieunomics Institute. Official forms and other third-party materials retain their respective status and ownership. Service interruptions and source errors can occur; report problems so we can correct them.
10. Your legal rights and future changes
Indiana law and applicable federal law govern these service terms, subject to any mandatory protections that apply to you. Nothing here excludes liability or waives a remedy that cannot lawfully be excluded, including applicable rights under Indiana’s Deceptive Consumer Sales Act (IC 24-5-0.5) and federal consumer-protection law. These terms impose no mandatory arbitration or class-action waiver.
Contact us first if you would like help resolving a problem; doing so does not prevent you from seeking legal advice, contacting a regulator or using available legal remedies. Agency decisions must be addressed through that agency’s applicable process and deadlines.
This version applies from the effective date shown above. Material changes will be identified here and brought to your attention when legally required. Changes do not retroactively alter completed purchases or earlier signing records. A new consent is required where the law requires one.

For service, privacy, accessible-format assistance, corrections or refunds, contact Milieu License, operated by Milieunomics Institute, at info@milieupower.com. Include the page or application reference and what you need. You do not need to buy a service to ask for help or exercise a privacy right.
