1Open the current resourceOutside step
Select only the permits required for your event. Use current agency forms for food, alcohol, amusement or entertainment, and tents instead of submitting the historical appendix bundle as one application.
- The county provides online applications for food, mobile, temporary food, and event plan review. The county process and payment are separate from the city application. Temporary-food applications must be completed at least seven days before the event.
- Obtain required local law-enforcement authorization first, then use the ATC online temporary beer/wine application at least five full business days before the event. Historical paper instructions are not the current process.
- Use the current IDHS Services Gateway for applicable amusement, entertainment, and tent permitting. Provide the required plans and anchorage documents and complete inspection. IDHS does not accept mailed payments.

This resource is not submitted as a standalone city application. Use the related form or responsible agency’s current process.
- Select applicable agency-specific form; this 23-page appendix bundle is not one application.
- Follow each component permit authority and signatures independently.
- Appendix F email kmyers@mishawaka.in.gov requests information on how to formally submit Board request; not a verified filing mailbox.
- No global email/mail/check destination; state ATC payment instructions must not be treated as City payee.
3Track completionOutside step
Confirm both city payment and the reservation with Parks. An application or email receipt alone does not hold the requested facility or date.
